VERY IMPORTANT! Please note that you have seven REQUIRED forms to complete before you become a client of Deirdre Haynes Counseling Services.

If your forms are not completed at least 24 hours prior to your appointment, then your appointment will be canceled.

This time frame allows Deirdre Haynes Counseling Services to check over your forms, contact your insurance provider if necessary, and to ensure that there are no financial surprises on the day of your appointment. This applies to EAP clients as well.
