Once you have logged in, you will be able to complete your intake forms and schedule future appointments.

b. Very Important! Your required intake forms must be completed by Monday at 8 am. If you schedule an appointment after Monday (i.e. during the same week), all intake forms must be completed by 8 am the day BEFORE your scheduled appointment so that there is time to verify and review your information. Once you complete your required intake forms, the therapist will meet with you via video or phone call at your scheduled date and time.
